Book the appropriate NEW PATIENT APPOINTMENT and we’ll get you started! Please complete your paperwork at least 12 hours in advance so we can focus on you during your visit. Please call if you do not see your preferred time available. If we are unable to answer, please leave a message and we’ll get back with you as soon as possible.

PLEASE NOTE: We are a direct pay office. We do not accept insurance. We do not want insurance telling us what we can and cannot do for you. More importantly, this means our recommendations are based on what’s best for you, not what’s determined by an insurance company, and it allows us to offer a variety of affordable payment options so that entire families can get the care they need.

We accept cash, check, debit/credit cards (including HSA and FSA) as forms of payment.

At this time we are NOT currently accepting:

• New patients age 65 or older due to Medicare Guidelines. To find an office that currently accepts Medicare, visit here.

• New patients who are seeking care for injuries sustained in an AUTO or WORK ACCIDENT.

CANCELLATION POLICY:

To ensure we can provide the highest level of care and accommodate all patients, we kindly request at least 24 hours’ notice for any appointment cancellations or rescheduling. Appointments canceled or missed with less than 24 hours’ notice will be subject to a cancellation fee of $75.This policy allows us to offer your reserved time to other patients who may be waiting for care.
